The Minnesota Department of Revenue requires that we receive this form from all vendors whether you plan to make sales at the show or not.
*Please download and save the PDF form. This form requires a signature to be valid. Please complete the form and attach to the upload form on this page or fax to 763-529-1922 or you may be fined by the Department of Revenue.
FOOD SAMPLING PERMIT (click to download)
Required for any exhibitor serving, preparing or giving away a food item. All guidelines and requirements must be followed and a Short Term Food Permit must be submitted *Please download and save the PDF forms. Submit the Short Term Food Permit to Twin Cities Wedding Association (attach to form on this page).

To be completed only if you would like to use lit candles or Sterno in your exhibit. Exhibitor is responsible for all permit fees. *Return application to the "Fire Inspection Services"
