Exhibit Staffing Form
To be completed so that your exhibit staff can gain access to the showfloor through our security check-in.

Sales and Use Tax Form
REQUIRED FROM ALL EXHIBITORS even if you are not collecting money or making sales at the event.
*Please download and save the PDF form. This form requires a signature to be valid. Please complete the form and attach to the upload form on this page or fax to 763-529-1922 by October 4th, 2017 or you may be fined by the Department of Revenue.
Food Sampling Requirements / Guidelines & Short Term Food Permit
Required for any exhibitor serving, preparing or giving away a food item. All guidelines and requirements must be followed and Short Term Food Permit must be submitted no later than October 4th, 2017. TCWA will cover the permit fee on or prior to this date.
*Please download and save the PDF forms (6 pages total). Submit the Short Term Food Permit to Twin Cities Wedding Association (attach to form on this page) by October 4th, 2017.
Open Flame Permit
To be completed only if you would like to use lit candles or Sterno in your exhibit. Exhibitor is responsible for all permit fees.
*Return application to the "Fire Inspection Services" by October 4th, 2017.
If you need to rent tables, order electricity, or purchase a list of registered couples, please fill out the order form by October 4th, 2017. Items may be limited or only available during set up.
